Duplicates and phone calls
When two people share a household and both shop, the list problem is different from a large family. There is less to buy, but far more overlap.
One of you buys bread on the way home. The other buys bread on the way home. Now there is a lot of bread, and neither of you did anything wrong.
The second failure is the call from the aisle, asking which of the two nearly identical products was the one you liked. That is not a list problem, it is a specification problem.
What stops the duplicate problem

- Product level detail, so either of you can buy the other person choice correctly

Does it work if only one of us has an iPhone?
Not yet. The iOS app is out and the Android version is coming, and until it does both of you need an iPhone to share a list.
